Recording risk factors for disease

The following tips and resources may be useful when organising systems for your practice to recognise, record and highlight periodontal risk.

  • Collect information on the patient’s risk factors before the appointment on a dedicated periodontal risk factor inquiry form. 

  • Once an increased periodontal risk is recognised, flag this in your computer software so that the whole team is aware. 

  • If the discussion about risk factor takes place in the surgery, the dental nurse can record the details of the conversation while the clinician discusses the situation with the patient.

  • Use the risk assessment tool to:    
    • generate an individual risk profile to discuss with the patient  
    • record the risk profile and discussion in the case records
