Review periodontal charting

Review periodontal charting can be recorded to assess treatment response and when charting annually for a patient with a diagnosis of periodontitis (to check for disease control or recurrence).

For a periodontal review chart, all sites in the mouth should be examined but only those sites with probing pocket depths of 4 mm or greater and those site where there is bleeding or suppuration on probing need to be recorded. 

It is recommended, but not necessary, that other parameters are recorded.

  • Assess probing pocket depth along with bleeding and suppuration on probing, at 6 points around each tooth.
    • Record the pocket depth in the case records at those sites where there is a probing pocket depth of 4 mm or greater.
    • Record bleeding or suppuration on probing from the base of the pocket.

  • Examine the whole mouth. 
  • Consider the individual patient and whether it is useful to assess and record:
    • Gingival recession
    • Tooth mobility
    • Furcation involvement
